Millicent
Millicent’s journey starts with treatment for Scarlet Rot in Caelid and ends with a choice in Elphael. Helping her and betraying her lead to different talismans, and only helping her preserves the route to Miquella’s Needle.
From Southern Aeonia Swamp Bank, follow the road north through Sellia Gateway. Watch for the giant throwing rocks above the gateway. Gowry's wooden shack is off the road on the right, before the town.
Speak to Gowry and agree to help Millicent. Keep the large dog away from the shack so its attacks do not interrupt the conversation.
Go west into the Swamp of Aeonia toward the clearing enclosed by large pale branches. Activate Inner Aeonia nearby and defeat Commander O'Neil in the Heart of Aeonia for the broken Unalloyed Gold Needle.
Return the needle to Gowry and finish his dialogue. He gives Sellia's Secret and needs time to repair the needle. Rest or reload, then speak to him again for the repaired version.
Continue north into Sellia, Town of Sorcery. Activate Sellia Under-Stair on the southwestern edge and follow the main street north, watching for sorcerers appearing beside the buildings.
Climb the stairs at the far end toward the golden sapling and the sealed upper gate. The large roots beside these steps provide the way onto the roofs.
Near the sapling, look southwest through the broken railing for a root leading toward the nearest candle tower. Follow it, climb the tower ladder and interact with the flame at the top.
Return to the upper steps and use the root again, this time stepping onto the adjoining roofs. Work south and east across the roofs and branches toward the eastern tower. Use Torrent's double jump for the gaps and climb the ladder to light its flame.
From that eastern roof route, head back across the connecting roots and roofs toward the southern tower on the western side. The narrow approach has a corpse with a Stonesword Key. Climb the ladder and light the third flame.
Take the now-open upper gate beside the golden sapling to Sellia Backstreets grace. Follow the uphill road beyond town, watching for the rolling boulders and the enemies on the bends.
At the top, go to Church of the Plague on the eastern cliff. Watch for the two Kindred of Rot outside, enter through the broken wall and activate the grace.
Give Millicent the repaired Unalloyed Gold Needle. Rest at Church of the Plague, then speak to her after she recovers and finish her dialogue to receive Prosthesis-Wearer Heirloom.

From the grace, walk up the small hill immediately north. Millicent stands above the nearby road after her Caelid meetings.

Starting Site of Grace: Drainage Channel
From Drainage Channel, backtrack west through the tunnel and climb the ladder. Leave through the western doorway, then turn right up the northwestern slope and clear the pest at its crest.
Turn south-southwest onto the large root crossing the pit. Drop from its end toward the isolated Scarlet Rot pool. An Ulcerated Tree Spirit appears when you approach; this is the enemy needed for Millicent's final signs.
Defeat the Tree Spirit, rest and return to the high ground above the pool's northwestern edge. The gold sign helps Millicent against her four sisters; the nearby red sign challenges her instead. Read the prompt before confirming.

Repair the needle
Branch: Main quest
Speak to Gowry at the shack south of Sellia. Defeat Commander O’Neil in the Heart of Aeonia, give Gowry the broken Unalloyed Gold Needle, rest and collect the repaired needle.
Treat Millicent
Branch: Main quest
Give her the repaired needle at Church of the Plague. Rest and speak again after she recovers. Collect Prosthesis-Wearer Heirloom, then meet her during her visit to Gowry’s Shack.
Find a replacement arm
Branch: Main quest
Meet her north of Erdtree-Gazing Hill. Obtain Valkyrie’s Prosthesis from the chest in the northwestern Shaded Castle room guarded by a Cleanrot Knight. Give her the arm; defeating Elemer is unnecessary.
Windmill Heights
Branch: Main quest
Defeat the Godskin Apostle at the highest windmill in Dominula. Rest at Windmill Heights and finish Millicent’s dialogue there. Summoning her is optional.
Continue north
Branch: Main quest
Speak to her beside Ancient Snow Valley Ruins in the Mountaintops. Later, reach Prayer Room in Elphael and finish her conversation about Malenia.
Prepare the final choice
Branch: Main quest
From Drainage Channel, backtrack up the ladder and follow the roots to the isolated rot pool. Kill its Ulcerated Tree Spirit, rest and return to the high ledge above the pool. Read the two sign prompts carefully.
Help her fight the sisters
Branch: Assist Millicent
Choose the gold sign and keep Millicent alive while defeating her four sisters. After winning, approach her beside the pool without jumping onto her. Finish her dialogue, rest and collect the returned Unalloyed Gold Needle.
Exchange the returned needle
Branch: Assist Millicent
Defeat Malenia, reload and interact with the scarlet flower in her boss arena while carrying the returned needle. Receive Miquella’s Needle and a Somber Ancient Dragon Smithing Stone.
Challenge her instead
Branch: Betray Millicent
Choose the red sign to fight against Millicent and obtain Millicent’s Prosthesis. This closes the returned-needle route. Killing her after she has received the arm and reached Windmill Heights is another hostile route.
Optional boss assistance
Branch: Cooperation
After treatment and her visit to Gowry’s Shack, she can help against Magma Wyrm Makar. After giving her the prosthesis and progressing her meetings, check for her signs at Dominula’s Godskin Apostle, the capital Draconic Tree Sentinel and the Forbidden Lands Black Blade Kindred.

The needle can remove the Frenzied Flame inside Placidusax’s arena.
Notes
The Valkyrie’s Prosthesis quest arm and Millicent’s Prosthesis talisman are different items.
Her swamp invasion is a separate encounter. Defeating that invader does not replace treating her in the church.
Her quest can still be completed after Malenia. Defeating Malenia does not require you to betray Millicent.
After the gold-sign battle, let her final conversation finish naturally. Attacking her to force another talisman can cost the needle.
The flower in Malenia’s boss arena is the exchange point. The separate orange flower outside that arena is not.
Her optional boss summon signs are assistance, not requirements to unlock the final choice.
A summoned ally falling in a boss attempt does not kill the ordinary quest character. These gold-sign fights are optional for her final Elphael choice.
