Millicent’s Sisters

Mary, Maureen, Amy and Polyanna are the four sisters fought during Millicent’s final quest choice. The sign you choose decides which side you join.

Starting Site of Grace: Drainage Channel

From Drainage Channel, backtrack west through the tunnel and climb the ladder. Leave through the western doorway, then turn right up the northwestern slope and clear the pest at its crest.
Turn south-southwest onto the large root crossing the pit. Drop from its end toward the isolated Scarlet Rot pool. An Ulcerated Tree Spirit appears when you approach; this is the enemy needed for Millicent's final signs.
Defeat the Tree Spirit, rest and return to the high ground above the pool's northwestern edge. The gold sign helps Millicent against her four sisters; the nearby red sign challenges her instead. Read the prompt before confirming.

Quest steps can include different outcomes. Read the branch, conditions and cutoffs before making a choice.

Unlock the signs
Branch: Main quest
Progress Millicent to Prayer Room, then kill the Ulcerated Tree Spirit in the isolated rot pool above Drainage Channel. Rest and inspect the two nearby signs.
Fight the sisters
Branch: Assist Millicent
Use the gold sign to enter Millicent’s world as her ally. Defeat all four while keeping her alive, then speak to her beside the pool in your own world.
Join the sisters
Branch: Betray Millicent
Use the red sign to challenge Millicent instead. This gives the alternative talisman and closes her returned-needle ending.

Mary uses a Halo Scythe, Maureen a Treespear, Amy paired curved swords and Polyanna Blade of Calling. Their different reach makes being surrounded dangerous.
Separate one opponent when possible, but watch Millicent so she is not overwhelmed elsewhere.
You can retry an unsuccessful summoned battle. A failed attempt does not automatically select the opposite branch.
The four sisters do not drop their carried weapons as a complete reward package.
After the gold battle, approach Millicent on foot. Do not jump onto her while trying to reach the final dialogue.
