Starting Site of Grace: Murkwater Cave

Enter the river north of Agheel Lake and follow it to Murkwater Cave in the western cliff. If Nerijus invades outside, deal with that encounter before entering.
Activate the cave grace, clear the bandits and enter the room with the chest. Opening the chest triggers Patches' fight.
When his health falls, he asks you to stop. Sparing him preserves his shop and quest; continuing the fight and killing him awards his bearing.
If you choose to kill him, collect the bearing and offer it to the Twin Maiden Husks at Roundtable Hold.

If you spared him, he can move through Scenic Isle, Volcano Manor and the Shaded Castle before returning to Murkwater Cave. Check his current quest location rather than assuming he has vanished.
