In-game description
Sinister greatsword fashioned from a giant's backbone.
Metes out wounds like a lopsided saw-blade, and restores some FP upon defeating an enemy.
Milos was undersized for a giant, and was viewed as sullied and terribly grotesque.
Skill
Lets out a horrific cursed scream that reduces all damage negation and status resistances for nearby foes. While active, strong attacks will change to a combo attack.
Where to find
Altus Plateau
Starting Site of Grace: Capital Rampart
Before you go
- Show the Dung Eater a Seedbed Curse, release him from the sewer gaol and read his message in Roundtable Hold.
- Enter Leyndell, Royal Capital after obtaining two Great Runes and defeating the Draconic Tree Sentinel outside its northeastern entrance. Cross the bridge, follow the building's stairs and activate East Capital Rampart.
- From East Capital Rampart, follow the outer walkway past the Oracle Envoys. Continue through the building at the end and take the lift down to the next part of the capital.
- Enter the large building ahead of the lift. Climb the ladder on the right, then the stairs to the upper room. Search the body tied to a chair for a Seedbed Curse. Collect this capital pickup before defeating Maliketh.
- Return to Roundtable Hold and enter the room beyond the Twin Maiden Husks. Speak to the red Dung Eater while carrying the Seedbed Curse and finish his dialogue to receive the Sewer-Gaol Key.
- In Leyndell, Royal Capital, start at Avenue Balcony. Descend the northwestern stairs and go through the door. Follow the railing west, then drop left onto the lower rooftops.
- Drop off the right side of the roof, turn back north and find the well behind the broken wall. Climb its ladder, open the gate in the lower passage and take the next ladder down.
- Follow the damp corridor northeast into the rat-filled sewer entrance. Go through the southeastern doorway and descend the ladder into the large Omen hallway.
- Head northeast along the hallway and enter the first room on the left, opposite the summoning statue. Activate Underground Roadside grace.
- Leave Underground Roadside and turn left into the main Omen hallway. Pass the Omen to the open floor grate and drop into the rat tunnel.
- Turn left and follow the tunnel past the rats. Continue into the passage with the large poison flowers, rather than turning into the side pipe used for the deeper sewer route. Climb the ladder at the far end.
- Watch for the Fingercreepers in the upper chamber. The Dung Eater's locked cell is on the right; open it with the Sewer-Gaol Key.
- Speak to the prisoner and tell him to leave his gaol. Return to his room at Roundtable Hold and read the message directing you to the outer moat. If Boggart has moved to the moat, advancing this part of the quest can kill him.
- Go to the water-filled outer moat north of Leyndell and defeat the Dung Eater's red invasion. This awards Sword of Milos and does not kill the prisoner. Return to Roundtable Hold and finish his new dialogue.
Subterranean Shunning-Grounds
Starting Site of Grace: Underground Roadside
Before you go
- Obtain Sewer-Gaol Key. Killing the prisoner ends his remaining quest branches.

- Return to Roundtable Hold and enter the room beyond the Twin Maiden Husks. Speak to the red Dung Eater while carrying the Seedbed Curse and finish his dialogue to receive the Sewer-Gaol Key.

- For the early hostile option, kill the real Dung Eater in the cell and collect Sword of Milos with his armor. This prevents his later Mending Rune and puppet branches. The red figure at Roundtable Hold is not this prisoner.
Notes
- Requires 15 Strength and 19 Dexterity and weighs 10. Physical damage scales more strongly with Dexterity, with useful Strength scaling. It has 55 innate blood-loss buildup and no Arcane scaling, so Arcane alone does not improve that bleed.
- Restores 5 FP when a nearby enemy dies while the sword remains in an active hand slot. You do not have to land the killing blow with this sword, so it can support another weapon or spell while held in the other hand.
- This is a kill reward rather than passive FP regeneration or FP on every hit. It cannot restore FP beyond your maximum, and a weapon merely sitting in an inactive equipment slot gives no benefit.
- Shriek of Milos costs 30 FP. The roar weakens nearby enemies and grants a 40-second weapon buff that increases physical attack power and changes heavy attacks into a brutal multi-swing chain.
- The roar itself deals stance pressure rather than a normal damaging sword hit. The changed heavy attacks use the sword and can apply its blood loss. An enemy's response to the roar does not guarantee that every follow-up will connect.
- Roar Medallion and Highland Axe strengthen the changed heavy attacks, rather than granting the same bonus to every light, running or jumping attack. Axe Talisman applies when an eligible heavy attack is fully charged.
- The weapon buff ends if you put the sword away, while the debuff already applied to an enemy can remain. The enemy debuff competes with Soul Stifler, Greyoll's Roar and the moon debuffs rather than stacking all their full effects.
- Upgrade with Somber Smithing Stones to +10. The skill and affinity are fixed, and outside grease or a weapon enchantment cannot be applied.
- Releasing the prisoner can lead to Boggart's death if Boggart has moved to Leyndell's moat. Finish his shopping and dialogue first. Killing the prisoner in his cell is a separate destructive alternative, while defeating the red invasion does not kill the prisoner or end his later quest.
- The roar’s weapon enhancement includes a 7.5% Physical attack-power increase and an additional Strength-scaling adjustment. It does not add five points to your character’s displayed Strength.
- The 40-second weapon enhancement and enemy debuff are separate effects. Switching away ends the weapon enhancement even if the enemy is still weakened.
