Increases direct damage by 10% for 20 seconds after Poison or Scarlet Rot triggers nearby. The bonus is the same in PvE and PvP and needs an actual status trigger, rather than a partly filled buildup meter.
These values are for this piece. Your total also depends on your other armor and active effects.
Poise & status resistance
Higher resistance lets more buildup accumulate before a status triggers.

The ten same-slot pieces closest in weight to Mushroom Crown. Lower weight is lighter; higher poise, negation and resistance provide more protection. Negation differences are percentage points.
Increases direct damage by 10% for 20 seconds after Poison or Scarlet Rot triggers nearby. The bonus is the same in PvE and PvP and needs an actual status trigger, rather than a partly filled buildup meter.
Your own Poison or Scarlet Rot can trigger it too. You can cure yourself afterward and keep the remaining buff time; staying poisoned for the whole duration is unnecessary.
Continued poison or rot damage does not keep refreshing the timer. Another nearby application can refresh the effect, but it does not stack several copies of the crown's bonus.

In-game description
Mushrooms found growing all over the body.
These overgrown mushrooms form a towering headpiece.
Raises attack power when something nearby suffers from poison or rot.
Long ago, great lords served the scarlet rot. Perhaps such fungal bodies served as their crowns.
Where to find
Corpse on a southeastern column
Lake of Rot
Starting Site of Grace: Lake of Rot Shoreside
Before you go
Reach Lake of Rot; the route below uses Ranni's quest.
Start at Caria Manor in northwestern Liurnia. Defeat Royal Knight Loretta and enter Three Sisters behind the manor. Visit Ranni at the top of Ranni's Rise and agree to serve her. Speak to the spirits downstairs, then return to Ranni to finish the conversation.
Defeat Starscourge Radahn at Redmane Castle. Return to southern Mistwood and find the new crater near Fort Haight, marked by floating rocks. Descend using its ledges and enter Nokron, Eternal City.
Work across Nokron's rooftops to the streets and continue to Mimic Tear. Defeat it and cross the long bridge beyond its arena. Leave the bridge on the left into the woods, then follow the cliff to Ancestral Woods grace.
Jump from the cliff beside that grace onto the roof below. Follow the rooftops and narrow building ledges into Night's Sacred Ground. Continue across the broken stone arch into the church, descend inside, and leave onto the lower street. Follow that street to the enormous seated figure and open the chest underneath it for Fingerslayer Blade.
Return the blade to Ranni at her tower. Before doing this, finish any remaining Seluvis rewards you want. Rest after speaking to Ranni, then visit Renna's Rise, the northern tower in Three Sisters. Climb it and use the sending gate to Ainsel River Main.
Collect Miniature Ranni from the coffin near where you arrive. Rest at Ainsel River Main and select Talk to miniature Ranni repeatedly until she answers. Finish her dialogue.
Follow the river through Uhl Palace Ruins, using cover against the hanging creature's projectiles. Continue through the ant tunnel to Nokstella. Follow the river at ground level through the city and take the lift in the small building at the far end to Nokstella Waterfall Basin.
Beyond Nokstella Waterfall Basin, follow the tunnel into the red-lit area with the Baleful Shadow and take the lift beyond it down to Lake of Rot. Activate Lake of Rot Shoreside before entering the water. Dark Moon Ring and the fight with Astel are not required for this pickup.
Cross toward the ruined building in the southeastern part of the lake. Stop on raised ground to manage buildup, and clear Scarlet Rot when it triggers. Do not roll through the lake, since coating your clothes can keep building rot after you leave it.
Watch for Basilisks around the ruin. South of the building, climb the fallen pillar that rests against a rock. From that rock, make a running jump north onto the leaning stone beam.
Jump from the beam to the next pillar and onto the top of the ruined wall. Follow the wall west, then look below for the column with a corpse hanging over its edge.
Drop carefully onto that column and loot Mushroom Crown. This helmet is separate from the four-piece pickup in Seethewater Cave.
Notes
Increases direct damage by 10% for 20 seconds after Poison or Scarlet Rot triggers nearby. The bonus is the same in PvE and PvP and needs an actual status trigger, rather than a partly filled buildup meter.
Your own Poison or Scarlet Rot can trigger it too. You can cure yourself afterward and keep the remaining buff time; staying poisoned for the whole duration is unnecessary.
Continued poison or rot damage does not keep refreshing the timer. Another nearby application can refresh the effect, but it does not stack several copies of the crown's bonus.
Stacks with Kindred of Rot's Exultation. Its 10% and the talisman's 20% PvE bonus multiply to 32% more direct damage while both are active, rather than adding to 30%.
Does not increase Poison or Scarlet Rot buildup or the ailment's damage over time. Its bonus improves your direct hits after a trigger; the armor does not poison enemies by itself.
Weighs 9.1 and gives 10 poise. It replaces your helmet, so you cannot wear it together with White Mask even though both have status-triggered damage bonuses.
The rest of the Mushroom outfit is not required for the effect. This crown cannot be altered and is a fixed Lake of Rot pickup, rather than part of the Seethewater Cave corpse.
